- Care for chronic, critically ill, and medically complex patients who require acute care over an extended period.
- Provid planning and delivery of direct and indirect patient care through the nursing process.
- Develop nursing care plans in coordination with patient, family, and interdisciplinary staff as necessary.
- Communicate changes in patient’s clinical condition with physicians, nursing supervisor, and co-workers as appropriate.
- Deliver intensive care to transition patients home or to a lower level of care as quickly and safely as possible.
Preceptorship at Kaiser Permanente, Ontario (May 2015 to July 2015, 252 hours) – Medical Surgical Telemetry Orthopedic Unit
- Applied the Nursing Process to provide quality, comprehensive care for up to 5 patients per shift under supervision of RN.
- Assessment: Admissions/discharges, head to toe assessments, vitals, and blood glucose, and other necessary care.
- Diagnosis: Made clinical judgements about clients’ needs and actual and potential health conditions.
- Outcomes/Planning: Created plans of care utilizing my clinical judgements and setting achievable and realistic goals. Documented all care and plans of care using charting software (MediTech 4 or Epic Hyperspace).

- Evaluation: Constant evaluation and re-evaluation of patient status and the effectiveness of nursing care was done throughout shifts and duration of Preceptorship. Provided patient education and coordinated patient care with physicians, staff nurses, physical therapy, laboratory, dietician, social work, and respiratory therapy.
